Please complete within the first 3 days of the dog being in your home and the 1st and 15th of every month while the foster dog is in care. This form will help us find the BEST forever home for your dog and aid us in helping you overcome any problems you may be having. Please CALL your appropriate coordinator with any questions or concerns.
Note: If the dog has just come out of board and train, foster reports are required every 7 days after the 1st report, unless given direction otherwise.

IT IS NEVER RECOMMENDED TO MESS WITH A DOG WHILE THEY ARE EATING OR HAVE HIGH VALUE ITEMS. PLEASE NEVER, EVER HANDLE THE DOG OR THE BOWL DURING THIS TIME. PICK UP ALL FOOD BOWLS AFTER MEAL TIMES. IF A MULTI-DOG HOUSEHOLD, FEED DOGS IN COMPLETELY SEPARATE SPACES AND PICK UP ALL BOWLS WHEN THEY ARE FINISHED EATING
